Directions

1. As soon as possible, prior to the Special Consultation with Dr. Rosen; Please print, fill out and fax (or download, complete and email) the:
1. Staff Evaluation
2. Stats (previous 12 months)
3. Service Routine form
2. Print and have your staff start keeping stats:
1. Stats Sheets Directions
2. Daily, weekly, monthly stats form
3. Month/year stats form (have your staff fax this one filled out at the end of each month to (808) 878-8384)
4. New Patient Retention stat sheets (monthly | yearly) – have your C.A. Fax these results to Rosen Coaching at the end of each month.
5. New Patient Tracking Sheet
6. Interpretation of Stats
3. Print the Pre-Coaching Preparation form and fill out 24 hours prior to each coaching session with Dr. Rosen. This is a great way to prioritize your thoughts, get the most out of each session and journal your progress.
